error with POAPPRV

hello all

I need help, because when making an approval of OC, this is a mistake in the "Book Document", the error that we make is the following:

Error Message ORA-06512: at line

what we did was

WF services stop and lift again, re-link leagues and enforceable.

rewind the OC was and still continues to make the same mistake.

can you help me please?

my application is 11.5.10.2
