Error Item ID (XXXXXXXXXXXXXXX) is not an item defined on the current page

Hi Folks

Recently upgraded form APEX 3.2 to 4.0 on an 11gR2 database.

I'm running into this error when navigating from one page with a mixture of hidden items and then selecting any TAB within the application.

If I navigate to these tabs directly from the login screen. there is no issue but if I select any tab having visited this particular page, I get

Error 	Item ID (6218131441531743) is not an item defined on the current page.

This was working perfectly in APEX 3.2.

Any suggestions?
