Error during create user workflow after upgrade

Hello,

We just upgrade from Oblix 6.1 to Oracle Access Manager 10.1.4.0.1
All went fine except for the Create User Workflow. When I now try to create a user I get the mysterious message that says "Win32 exception caught in ModuleMain".
Our workflow consists of 3 steps and this error occurs between step 1 and step 2. In the first step, we enter some details about the user and in step 2 we can enter some additional information about the user.

This workflow used to have custom stylesheets in the past but now we have not made any customizations to the stylesheet.

In the logfile oblog.log, I see the following message everytime I have the error above in my browser:
2008/01/22@14:26:08.734000 2948 1888 APP_BASE ERROR 0x00000848 data\module_main.cpp:828 "OIS_LOG_PASSED_ERROR" Cause^Win32 exception caught in ModuleMain

Can anybody help us on this error?

We run OAM on a Windows 2000 Advanced Server on IIS5 with Active Directory.
This workflow also uses a custom dll file but in our logfiles we see that the error occurs before it even reaches this dll file.
