Erroneous error msg: "The file could not be attached because it contains no data."

I'm getting this error message a lot all of a sudden even though the attached file is fine. No issues with the thumbnail. No issues with the upload as an attachment. No issues with including it in a form. Just an annoying error message.
There doesn't seem to be any consistency either. Sometimes the a file works fine, while another time the same file will throw the error message. Sometimes a particular control throws the error. Sometimes it doesn't. Sometimes an interview will only throw 1-2 errors. Sometimes dozens.
I've traced the addFile code and don't see any issues. I have no idea why the error message is being added.

I am using a styling extension on the container with the upload control to push the mime type into a hidden text control so that I can display the image in a form, but that doesn't appear to be interacting with addFile functionality that's sending the error message.
Anybody else ever seen this? Any alternative approaches to displaying images from a multi-extension upload in a rtf template? Any other debug options I might pursue other than opening a SR?
