entries in /etc/sudoers lost after reboot

I had updated the /etc/sudoers file and after reboot I see the file has been reverted back to its original form. This happens every time the server is rebooted.

I did check there are no logs being found in /var/log/secure other than authentication failures

However it has nothing to do with the reboot, but this is the only host which I see this issue. I have never come across such thing as of now.

OS version - OL 6.6 X86_64

sudo version - sudo-1.8.6p3-15.el6.x86_64

Any help please ?
