EAS & APS 11.1.2.4 : error while applying patches

hello ,

We have installed and configured EPM system 11.1.2.4

server 1 : shared services, calc manager, planning, EAS and APS

server 2 : Essbase server and essbase studio

server 3 : HFM , Financial Reporting, EPMA

server 4 : FDMEE

I have applied patches successfully on all server

but while applying patches for APS and EAS (11.1.2.4.005) I am getting error (PFA )

Opatch failed with error code 39

EAS and APS is installed and working fine on server
