Dynamic Action displays alert message twice - Why?

I've a dynamic action on field mfg_date which has a value of either 1 or 0. If 1 it displays a message "Invalid Date". When running the application, the message is displayed twice and I've to enter OK two times.

Is there a way to make sure the alert is displayed on once or Dynamic action only fires once? Am I mssing anything? Any help is appreciated. We are using APEX 4.0
