duplicate column name error on APEX 5.04

Hi maters,

I try to input a data into the IR table, but get the following error statement:

1 error has occurred

  • ORA-06550: line 43, column 2: PL/SQL: ORA-00957: duplicate column name ORA-06550: line 1, column 7: PL/SQL: SQL Statement ignored

How can i solve the error?
