Download my File giving error 404 ..... Again!

I followed the example how_to, changing Apex references to HTMLDB references to make things work :-)
All works fine until I try using the Custom Table. First I got the HTTP 403 error, and by looking in these threads found the fix using FLOWS_020100 and function www_flow_epg_include_mod_local, tried with accept local and open http connections.
I still get error 404 when I try to open the document, I have checked the file, and the Blob content does exist as does the ID number and file length. Also there are no errors shown in the apache web server.
