Documenting backup procedures and restore procedures

Are there any general best practices when it comes to documenting how your backup/restore processes work for your oracle databases?

I always fear that if only one admin understands how the backup/restore procedures works for business critical systems, that if they leave the organisation that someone filling their shoes may struggle to understand the process and then if a restore is required may not be able to complete the restore.

But not being a DBA myself, what sort of details should they document. What sort of details do you document in your organisations?

Also, how often do you test restores per database?
