Document Reference Duplicate Submission

Expected Result:

I expect that the DocumentReference submission should be fully successful. However, we are noticing our system submitting duplicate requests. When we submit the request, the first request gives 201, but is missing subject and body. When we resubmit the request, the subject and body is missing (second request is only one second later). I'm curious to understand what could be preventing the body and subject to not be going through successfully.

Actual Result:

This request results in a DocumentReference post with 201, but the body of the text and subject doesn't propagate (unsuccessful):
