Do we have to pay licensing & support cost every year for DB licenses?

According to the Oracle Global Price list (https://www.oracle.com/assets/technology-price-list-070617.pdf )
Suppose we have 300 different (named) users who use this DB. We obviously have to go for a Processor license.
A SE2 DB Processor License is $17,500 .
The Software Update License & Support is $3,850.
So total cost is $21,350.
My questions are
(1.) Is is this per-year licensing cost or one-time only? I.e. Do we have to pay Oracle $21,350 every year??
(2.) These SW Update and License and support: What does it consist of?? I.e. If we had a 11g DB, will we get the 11g r2 free if we had this support cost paid? What about Oracle Support logins?? Do we get access to that to read bug-reports etc?
(3.) If licensing and support cost is NOT on a per-year basis when do have to pay these 2 costs (licensing & support) again?? Is it when we upgrade to a major version, like from 11g SE2 to 12g SE2??
(4.) Is this how all other Oracle products are licensed (i.e. not per-year but one-time only)?
