Displaying warning message when deleting a record

Hi All,

I have a Custom Apps form. When deleting a record, i want to display a message like 'Do you want to delete?' with YES and CENCEL buttons. If the user presses YES then the record should be deleted. If CANCEL then the record should not be deleted.

What is the trigger to be used and sample code to accomplish this? This is very urgent.
