Developer toolbar missing

I can no longer see the developer toolbar in my apex application. I am logging in with as an admin user and used to be able to see it. Is there a way to turn it on?

Also, on a related note, what URL format should I use to go directly to edit mode for a page?, if that's even possible? I am using the embedded pl/sql gateway, so my URLs look like http://hostname:8888/apex/f?p=123:101:1369073988092846.
