DB growth of last 3 month

Hi,

I am looking for script that would provide monthly wise database growth along with columns 'Actul DB size(GB)' ,' Available DB size(GB)', 'Used DB size(GB)', 'Used Percentage', 'Available Percentage DB growth (May)'.

Could you please provide me script to get data on monthly wise. I am mainly looking for last 3 month DB growth.
