Database hangs with "useg scan erroring out with error e:12751" message

Hi,

My test database always hangs. Users are having a hard time connecting to the DB. Alert log had the following error message.

minact-scn: got error during useg scan e:12751 usn:21

minact-scn: useg scan erroring out with error e:12751

When a datafile creation statement is run, though the file is created in the file system, the statement doesn't return.
