APEX

Seems I posted my first thread for this in th wrong place, so here's a second attempt...

I have created a variety of forms and reports in an apex application. They all 'work' (ie they display correctly) but none of the forms will accept a new data entry.
No error message is displayed.
I've done everything like setting the button redirect back to the same page, clearing the cache for the page etc.
What am I doing wrong?
