configure 6400 scsi controler

I have attached a 6400 scsi controler 6400 on my HP DL 380 G5 running Oracle Unbreakable Linux 4 to make a backup to a Dell Power Vault 100 T tape drive.
1. On the first screen BIOS detection it shows that theres is a 6400 scsi controler
2. Shows that has detected a TAPE or CD-ROM device because i already conected the Tape Drive to it;
3. The first time i booted the kudzo showed up and asked me if i wanted to configure the new device i typed yes.
after that nothing els happen, i logon and there is no device on the system. Here are the outputs of the commands i tryed to check if the device is there or not:
