Common Access Card (CAC) APEX Authentication

Has anyone done anything with Smart-card and Common Access Card (CAC) systems in APEX? I have a requirement to CAC enable APEX apps using the clients certificate. I am hoping someone has done something similar and can share their lessons learned.
