Cloud Control 12.1.0.4 couldn't display plugin

Cloud Control Version 12.1.0.4 Getting couldn't load plugin when accessing the database performance page.

I have 30 databases being monitored with Cloud Control 12.1.0.4. Both 12c and 11.2.0.4 Enterprise database editions. I am getting Couldn't Load plugins from the database Performance => Performance Home Page and the Top Activity Page.

How do I resolve this issue?
