Clearing or refreshing the form fields

the dialoge box is a okcancle box...
its has a form to update the values in table....
all the form controis are drop down or input text
once i select the value in drop down or type somthing in text box it never goes of the form till i reload the application...
i trie to creal the cache of data control but nothing works when i submitt the form
please help me out with the situation....
