change of url http://localhost:9001/forms/frmservlet to http://localhost:9001/name

Hi,

I am working on web logic 11g and forms and report 11g. Every time to run the form I am using http://localhost:9001/forms/frmservlet. I don't want forms/frmservlet I need name ex: http://localhost:9001/name . Can we change like this.

   Help will be appreciable.
