CDG-01281 ERROR: Oracle Forms binary file for VAL has NOT been created

hello
i have a problem with this error message
CDG-01281 ERROR: Module VAL: Oracle Forms binary file for VAL has NOT been created

i`m trying to build a simple form for query and insert, but when i generate it i get the message above.
because it does not create a forms binary file there is no possibility to open form builder and check there what is wrong
other form modules are working fine, only this is causing headaches.
