CCDA with complete Progress Note

Workflow or API calls:

  • Cozeva is using /binary/$autogen-ccd-if resource with both 'patient' and 'encounter' input parameters populated.

Background Information:

  • We noticed in the CCDs for a partricular client, majority of the patients would not have the 'Progress Note'-type section, which includes the Chief Complaint, History of Present Illness, etc - that is documented in the actual encounter clinical note.
  • For some cases, we received the data in the 'Physician Note' section, when the encounter note is placed in the 'Physician Note' folder. However, when the notes are located in other folders, the note is not present.
  • We have tried using different options, starting from the DocumentReference resource (both with the 'patient' and 'encounter' parameters, /DocumentReference/$docref endpoint, looking at all 'type' and 'contentType' - including XML, PDF, and HTML) and still did not find the Progress Note populated at all times, for encounters that are completed.

Questions:

  • Is this a configuration option that needs to be set for each 'folder' in CommonSpirit's Oracle Health for the CCD export data?
  • Are there other resource/parameters we should be looking into to get the complete Progress Note documentation?
