Cannot connect with Host credentials

Hi All,
I have been followed everything I found under Metalink, and here..but it remain the same. I cannot connect to my DB by OEM. My host credentials cannot being recognized.
"Error: Connection to host as user xxx failed. Failed to create command process"

Any idea?
