Calling Reports from Forms 12c

Hello,

I'm trying to call a report from Forms 12c using the same code in this link.

This was working with 11g, but in 12c it is not working showing error "Unable to run report".

I think that I missed something because Reports port is 9002, and Forms port is 9001. This was not the case in 11g and I didn't find in the installation instructions anything to link the two.

Please note that both reports and forms are created in the same instance on the same server.
