Bug// File Brows does not work in APEX 4.1.1

Hi guys,
I have just installed APEX 4.1.1 on Oracle 11g xe R2. on window server 2008 WEB SERVER machine. with APEX Listener 1.1.3. deployed on Glassfish 3.1.2. and user Goolge chrome..

File browse item does not work... whether on my application (when the value is required ) or on Application builder.e.g when I tried to import an application:
I chose the file, then clicked NEXT, but, I get this error:

1 error has occurred
File must be selected to upload. (Go to error)

** One thing to mention, when I installed APEX, I did not upload the images

@apxldimg.sql

And that for I want to work on Glassfish, and suppose no need to upload the images into the DB...
