I want to download and install Application Server(6) On Windows Nt

Above details are of the app server which is already been installed long back but the problem with me is that the installation cd is not available so i want to download and install the same but even i dont know the exact version of the App server so can any body tell me the exaction version of the app server by seeing the above details and also the link from where i can download the software.
