APEX Survey Builder returns blank page

I'm using Survey Builder with Apex 5.0 and Oracle Database 12c but I'm having issues some apex public URL's. Sometimes browser shows a blank page and problem dissappears after some minutes / refreshing.

Here are both situations:

URL is of the type: http://myurl:8090/ords/f?p=100:Q::AABG:::code:TIDZPTATHREPCMNI

First load : BLANK PAGE

I tried refreshing but it continues returning a blank page. Then I tried after 20-30 minutes and there was the correct output. Here are some screenshots of same URL:

Does anyone knows why this is happening?

UPDATE: Here is the log of Tomcat:

When URL works:
