APEX_MAIL Setup

I have looked through the Help and the online manual and I am not seeing:

1. How to tell if APEX_MAIL and the underlying UTL_SMTP package is installed.
2. If it is not installed, how to install it (where is it, etc.). In http://download-west.oracle.com/docs/cd/B19306_01/appdev.102/b14258/u_smtp.htm#ARPLS074 , we are told all about the UTL_SMTP package usage, but nothing about how to install it.

Under Configuring Email Environment Settings, I am instructed to

1. Click Manage Service (I have Manages Services, plural)
2. Click Manage Environment Settings (not present in my installation, Application Express 2.2.1.00.04)

Really appreciate any guidance, pointing to a good document, or anything.
