APEX and SSO Integration... LOGOUT issues

We are integrating an APEX 3.2.0.00.27 application with Oracle SSO. We have managed to configure the login process with SSO successfully for authentication and we have authorization working. We are trying to get the LOGOUT button working properly in APEX. Here are the 2 issues we have:

1. If you click the APEX "LOGOUT" button on the top right it takes you to the SSO LOGOUT page, but it displays a broken check mark box on the SSO LOGOUT page. All our other SSO apps properly display a check mark (eBusiness 11.5.10.2, Discoverer Viewer, Self-Service Console). However, with this APEX application, we get a broken icon and the SSO page says to "close the browser" to logout. We would like this to work properly and fix whatever is preventing the check mark from being rendered. I don't know what internally Oracle SSO is checking to render this icon or not.

2. The SSO page displays a RETURN button. Normally, this returns you to the SSO login page to enter your SSO username and password. However, when we click the RETURN button, we get:

ORA-01403: no data found
Error
Error with SSO Log-in Credentials
OK

Any ideas if this is misconfiguration in APEX? Do we have the "Public Page" misconfigured? We really don't want end users (5,000) to see any errors messages when they use this application.
