APEX 5 Login Page: include logo on the top left corner

Hi All:

I am using APEX5.1.2. I am looking for instruction to include a company logo on top-left corner on the login page (e.g. page# 101). Basically, I want to build my application login page also same like the Login page of the APEX.

There are many example of how to build a nice login page I can find. However, I can't find an example including logo on the top left corner.
