APEX 3.0.1 - missing breadcrumbs and page headers

Somehow the breadcrumbs and the page headers for my application have disappeared. Is there a way to get them back? I see something remaining under 'Breadcrumbs' and I think I can fix that, but I'm unsure as to what to do about the page headers. Any suggestions would be greatly appreciated.
