Apache.exe crashing - AS10.1.2.2.0 - Middle Tier Forms/Reports/Discoverer

We have a Windows 2003 R2 32-bit server with 32.0GB of memory and multiple processor cores.

The box runs AS10.1.2.2.0 with both Infrastructure and MiddleTier instances.

We have had a Forms / Reports application deployed for a while and been gradually increasing the number of users using the system.

Once we get over a certain number of users on the system, the Apache.exe process crashes and logs the following error in the system event log: 'Application popup: Apache.exe -
Application Error : The instruction at "0x7c82be3e" referenced memory at
"0xfffffff8". The memory could not be "read".'

Has anybody experienced anything similar?
