An element with the same key but a different value already exists

Hi, we are working on DRM version 11.1.2.4.321. I am trying to create a new hierarchy and I am getting the error "An element with the same key but a different value already exists".

In the application, I have only one version and this is not a duplicate hierarchy. Any information on this error please...........
