Add a comment button in the end of row in Interactive Report

I am Apex Version 5. In my Interactive Report in the last column i need a "Add Comment" Button. How to Add this Button. And also if am Clicking on that it should opens a Modal Page and i should add a Comment and those Comment should get displayed below the Add Comment Button in my Report. How to do this.
