Activex Problem in Forms 6i

I want to embed ActiveX control of VLC Media Player using following steps:

1. Create item with type activex control.

2. insert control VLC ACTIVEX PLUGIN

but when i run the form the ActiveX item disappears.

kindly suggest me the problem exactly where it is.
