Abnormal termination, Error Code: C0000005 ACCESS_VIOLATION

Hi All:

I am using Oracle Database 10g Enterprise Edition Release 10.1.0.2.0
and Developer Forms [32 Bit] Version 6.0.8.23.2.

I have created a menu. When I call this menu in a form and execute this from either Form Builder or Form Run Time, form executed, but immediately closed when I select any Menu option.

I have seen that every time when I tried to execute my form system generate a dump file. I found the following error in this dump file:
