100% swap, how to reduce its usage?

Hi All,
Hoping you could offer some suggestions.

RAM --> 256 GB
oracle 11.2.0.2 on RHEL 5.7
has 2 databases on this database server, with each allocated 50GB.(MEMORY_MAX_TARGET=MEMORY_TARGET=50G)

free -h
lnx133:/app/oracle/product/agent11.2:>free -g
total used free shared buffers cached
Mem: 251 34 217 0 0 31
-/+ buffers/cache: 3 248
Swap: 17 17 0

Why does this command show 34GB used for RAM? & also if RAM is available why has swap space jumped upto 100%? this started happening last week.

vmstat output is as follows

swap in/out is 0 from the output? please help me connect these dots.
